    The therapeutic use of homework assignments is an integral feature of Cognitive Behaviour Therapy (CBT) and is believed to be important in producing and maintaining client treatment gains. While there is empirical and theoretical support suggesting that homework completion may be associated with improved outcome, few studies have directly investigated specific therapist behaviours used in the integration of homework into practice. Similarly, research on client homework completion has, by far, focused solely on the quantity of homework completion rather than on any other factors found to be associated with increased client engagement with homework assignments. The recently developed Homework Adherence and Competence Scale (HAACS) was designed to specifically assess therapist competence and adherence to recommended homework behaviours used to administer homework. As a new measure of client homework completion, the Homework Rating Scale-II (HRS-II), extends the assessment of client homework completion beyond quantity only and provides a measure of the cognitive and behavioural determinants of client homework completion. The present study presents a psychometric evaluation of both the HAACS and HRS-II when rated by independent observers. Further, the present study sought to examine therapist differences in adherence and competence in administrating homework as well as investigating the temporal pattern of client homework completion

    Disagreement between correlations of quantum mechanics and stochastic electrodynamics in the damped parametric oscillator

    Intracavity and external third order correlations in the damped nondegenerate parametric oscillator are calculated for quantum mechanics and stochastic electrodynamics (SED), a semiclassical theory. The two theories yield greatly different results, with the correlations of quantum mechanics being cubic in the system's nonlinear coupling constant and those of SED being linear in the same constant. In particular, differences between the two theories are present in at least a mesoscopic regime. They also exist when realistic damping is included. Such differences illustrate distinctions between quantum mechanics and a hidden variable theory for continuous variables.Comment: accepted by PR

    Persistent hypertriglyceridemia in statin-treated patients with type 2 diabetes mellitus

    Purpose: This paper reports the results of an audit that assessed the prevalence of residual hypertriglyceridemia and the potential need for intensified management among patients with statin-treated type 2 diabetes mellitus (T2DM) in primary care in the UK. Patients and methods: A cross-sectional, observational, systematic audit of patients with diagnosed diabetes from 40 primary care practices was undertaken. The audit collected basic demographic information and data on prescriptions issued during the preceding 4 months. T2DM patients were stratified according to the proportion that attained European Society of Cardiology treatment targets. Results: The audit collected data from 14,652 patients with diagnosed diabetes: 89.5% (n = 13,108) of the total cohort had T2DM. Of the people with T2DM, 22.2% (2916) were not currently receiving lipid-lowering therapy. Up to approximately 80% of these people showed evidence of dyslipidemia. Among the group that received lipid-lowering therapy, 94.7% (9647) were on statin monotherapy, which was usually simvastatin (69.5% of patients receiving statin monotherapy; 6707). The currently available statins were prescribed, with the most common dose being 40 mg simvastatin (44.2%; 4267). Irrespective of the statin used, around half of the patients receiving statin monotherapy did not attain the European Society of Cardiology treatment targets for triglycerides, low-density lipoprotein, high-density lipoprotein, and total cholesterol. Conclusion: T2DM patients managed in UK primary care commonly show persistent lipid abnormalities. Clinicians need to optimize compliance with lipid-lowering and other medications. Clinicians also need to consider intensifying statin regimens, prescribing additional lipid-modifying therapies, and specific treatments aimed at triglyceride lowering to improve dyslipidemia control in statin-treated patients with T2DM

    We show that a universal set of gates for quantum computation with optics can be quantum teleported through the use of EPR entangled states, homodyne detection, and linear optics and squeezing operations conditioned on measurement outcomes. This scheme may be used for fault-tolerant quantum computation in any optical scheme (qubit or continuous variable).     System Design for a Long-Line Quantum Repeater

    We present a new control algorithm and system design for a network of quantum repeaters, and outline the end-to-end protocol architecture. Such a network will create long-distance quantum states, supporting quantum key distribution as well as distributed quantum computation. Quantum repeaters improve the reduction of quantum-communication throughput with distance from exponential to polynomial. Because a quantum state cannot be copied, a quantum repeater is not a signal amplifier, but rather executes algorithms for quantum teleportation in conjunction with a specialized type of quantum error correction called purification to raise the fidelity of the quantum states. We introduce our banded purification scheme, which is especially effective when the fidelity of coupled qubits is low, improving the prospects for experimental realization of such systems. The resulting throughput is calculated via detailed simulations of a long line composed of shorter hops.     Signatures of the collapse and revival of a spin Schr\"{o}dinger cat state in a continuously monitored field mode

    We study the effects of continuous measurement of the field mode during the collapse and revival of spin Schr\"{o}dinger cat states in the Tavis-Cummings model of N qubits (two-level quantum systems) coupled to a field mode. We show that a compromise between relatively weak and relatively strong continuous measurement will not completely destroy the collapse and revival dynamics while still providing enough signal-to-noise resolution to identify the signatures of the process in the measurement record. This type of measurement would in principle allow the verification of the occurrence of the collapse and revival of a spin Schr\"{o}dinger cat state.Comment: 5 pages, 2 figure
